How to Become a Member

Download the Membership Application Form and Questionnaire, complete and sign them.

The application form must be signed by an existing GP member before being mailed with the questionnaire to the Division at:

Mid North Coast Division of General Practice
PO Box 920
Coffs Harbour  NSW  2450

On receipt the application will be presented to the Board for their approval at the next Board Meeting.

Once your membership has been approved by the Board you will receive an acceptance letter.
