In-house multidisciplinary care planning meetings were successfully trialled as part of the Mid North Coast Rural Palliative Care Project 2004-6.
The Toolkit developed for this purpose was revised and enhanced in 2007 by the Aged Care GP Panels Initiative and Integrated Network Palliative Care Project, in consultation with the aged care multidisciplinary team meeting coordinators to enable their individual and collective experience to be incorporated into this second edition. The Toolkit outlines the process for multidisciplinary team meetings to assist residential aged care facilities who wish to implement them. Individual tools are provided in order of their application to the process, from scheduling and gathering relevant information to running the meeting, documenting and distributing the plan and collecting outcomes data.
