Shared Care Wound Management
This project is a collaboration between the North Coast Area Health Service and the Mid North Coast Division of General Practice.
The overall aim of the project is to develop and implement a systematic and shared care model of wound management that will include the patient, their carers, GPs, Community Nurses, Practice Nurses, Residential Aged Care facilities (RACF), pharmacists, podiatrists, Indigenous Health Workers and other health professionals.
