Chronic Disease Management
- Have recruited practices to set up practice registers & recall/reminder systems to the APCC and Chronic Disease Collaborative program
- Install and maintain the Canning Extraction Tool and provide training regarding its use to enable the monthly collection of clinical performance indicators from practices.
- Assist practices (APCC practices included) to develop strategies to improve these statistics.
- Division Information System (ChilliDB) maintains record of all stats.
