Visting Specialist Services
In 2004, the Mid North Coast Division of General Practice established an Adult Diabetes Endocrinology Clinic with funding under the Commonwealth More Specialist Outreach Assistance Program (MSOAP), to support a visiting Endocrinologist outreaching from Sydney. As part of a collaborative effort, Diabetes Allied Health Service support for the clinic was provided by the Mid North Coast Area Health Service. Dr Vernon Naidu, provided the essential visiting services, supported by members of the Mid North Coast (Coffs Harbour Health Campus) Diabetes team.
The Adult Diabetes Clinic (Endocrinology) was built around an integrated model of care, involving AHS, Allied Health, GP and Specialist Care Coordination and Education. The Clinic proved to be very successful and was maintained until 2006, when the location of a full time Private Endocrinologist to the Coffs Coast, Dr Sultan Linjawi resulted in this specialist gap being filled and the visiting specialist clinic was no longer required.
Prior to the establishment of a full time Endocrinology service at Coffs Harbour, the Division built upon the success of the visiting Adult Diabetes Endocrinology Service, by submitting a successful application for funding under the MSOAP program to bring another visiting Endocrinologist, Dr Nadia Tejani, to the area. Dr Tejani, provided essential Endocrinology services that complemented the existing service. With the provision of full time services by resident Endocronologist, Dr Linjawi, there was little need to continue the visiting services, and Dr Tejani's visiting service concluded in early 2007.
The Division has consulted with Dr Linjawi on the development of an integrated model of Diabetes Care, involving GPs, Specialists, and Allied Health Services, and commited to ongoing work in that direction. The Division has partnered with member GP practices to establish GP Diabetes Clinics, supported by Diabetes Educators provided by the Division, and built upon the concept of Integrated Diabetes Care, linking GP care with Area Health Service and Private Diabetes Allied Health workers, Endocrinologists, Specialists, and Practice Nurses.
