Palliative Care Multidisciplinary Team (MDT)
The Palliative Care Multidisciplinary Team (MDT) Meeting provides a forum for:
- Case conferencing to plan care for patients (who have given consent) with complex palliative care needs or when multiple providers are involved
- Discharge case conferencing for patients with palliative care needs to support a smoother transition between care settings (eg hospital to hospital, hospital to home/aged care facility)
- Learning about management of palliative care symptoms and care issues
- Sharing knowledge
- Meeting and networking with other care providers
- Improving communication and strengthen professional relationships
MDT Meeting details:
Held on alternate TUESDAYS, from 8am at Baringa Private Hospital
Choice of attending in person or via teleconference
Case conference summary letter is sent to care providers within 24 hours
