Rural Primary Health Service - Diabetes Education

The Mid North Coast Division of General Practice (MNCDGP) employs three Registered Nurses who are Credentialed Diabetes Educators: Lynette Dalgliesh, Sheryl Harper & Denise Maxwell–Hopper. They provide individual diabetes education to diabetic patients referred by their GP at the GP practice.  Currently 20 GP practices access this service in the Division area. This is a free service to both GP practices and patients. Client feedback regarding this service is extremely positive. This is a free and confidential service. It is a team based approach with the doctor as team leader.

 Step 1
Referral is from the GP, either typed into the progress notes or in ‘letters’.

Step 2
Clients bring any relevant diabetes supplies (e.g. meter, Insulin, diary) to the Diabetes Educator appointment.

Step 3
After the appointment, a Diabetes Education Service Report will be forwarded to the doctor or reported into progress notes.

Step 4
Some patients will need a follow up appointment with the Diabetes Educator after an agreed period of time. These patients will be recalled for this appointment.
